Transcript for Scenario 2: Referrals

This video transcript is tutorial on how to use the My Dementia Companion PRO to manage referrals and follow up using Sharing, so you can improve care, build relationships and even business development.

Welcome to My Dementia Companion PRO tutorial on how to use the tool in your day-to-day work when you receive a referral.

 

Gathering some background information will help you feel more confident and give the family reassurance you can support them on their journey.

 

PRO Tip: set your client up first using the 'Client & Carer' tab on the left-hand side menu. And don't forget to ensure you have the correct 'State/Territory' selected to match your client's location.

 

In this My Aged Care referral example, you can see several words that you can start searching on for today's tutorial. We're going to focus on 'driving', which can be a big issue for the client and their family, and often comes up long before diagnosis. 

 

Once you search or select 'driving' , start working through the sections that are  relevant when you select a Section you can see more detailed information, including evidence-based and up-to-date videos, links to practical care strategies and even Tips from other carers. These can help you prepare for more practical strategies.

You can click on the heading to collapse and see other related Sections.

This will help you prepare for your first visit.

 

After your first visit, you can 'Share', 'Save' or 'Mark Done' as a follow-up and to evidence the support you're providing to your client's personal dementia journey. This also helps to build your relationship with them and even helps with business development.

 

'Sharing' is as easy as selecting what you want to share. type a heading or edit a pre-populated message, cc another family member or a care worker and then just hit 'Send'.

 

Clients/ carers don't need to register to access links shared by you nor create their profile in My Dementia Companion.

 

Emailing and sharing personalised information with no additional work on your part to prospective or existing clients not only makes them feel important and receive quality care, it records all the amazing work you do.

 

After you send that initial email, any further correspondence is stored in your email system and not in the PRO.

 

Anything shared from the PRO has a full history for you to access via the 1) Journey map, 2) 'Shared' tab, including if the client opened what you have shared, or 3) the 'Client & Carer' tab's history 'clock' icon. This history helps evidence care management, and reduce or resolve any serious complaints.

Note that the PRO is an important tool and ensures teams are sourcing evidence-based, quality and up-to-date information from the same place. The PRO app should be used as a prompt to support professional training and judgment. Expand and collapse Sections by clicking on the heading to help you orientate yourself.
