Welcome to My Dementia Companion PRO tutorial on how to use the tool to develop a care plan from scratch or review your existing care plan. Gathering some background information will help you feel more confident and give the family reassurance that you can support them on their journey.

 

Let's take 'hallucination' as an example, which is common, requires support and strategies for carers and the care teams.

 

Once you search or select a relevant hallucination result, start working through the Sections that are relevant. When you make your selection, you can see more detailed information, including videos, links and even Care Tips, which can give you more practical strategies.

 

Using the client's goals, assessed needs and the strategies, you've identified you can develop a care plan that will plan for this know changed behavior to support the care worker and provide a better client outcome to a critically needed service, such as respite.

 

Reviewing of care and services can also involve ad hoc or unplanned events that could easily be reviewed using the tool to identify strategies. This example shows an unmet need of to be able to navigate around the home.

 

A quick search for 'signs' locates a link for you to share download or purchase dementia friendly signs to help. It's that simple. Don't forget to update your progress notes of the wonderful work you're doing.

 

Starting with a blank canvas, limited referral (or assessment) information or maybe you want to do a full review?

 

No problem, use the 'Planning' tab in the menu to step through relevant health Domains based on the Department of Health and Age Care. This feature will prompt you to ask questions and identify any unmet needs, so nothing is missed.

 

An example of someone needing support with Activities of Daily Living, simply click on that Domain and all relevant Topics will appear to guide you, including early stage strategies such as cooking and meal preparation, memory aids and more.

 

Don't forget to Share or print any relevant information as a follow-up.

 

Note that the PRO is an important tool and ensures teams are sourcing evidence-based, quality and up-to-date information from the same place. The PRO app should be used as a prompt to support professional training and judgment. Expand and collapse Sections by clicking on the heading to help you orientate yourself.
