Why: Dementia is cited 37 times in the new Strengthened Standards. We care that our clients have the tools to live well and make their own choices based on their individual needs.

What: It is a digital tool that helps to guide their individual journey through early, middle and late stages of dementia. It’s not training, it empowers and builds skills to navigate.

How: State and National specific resources, including information, forms, links etc. on important topics, such as Understanding Dementia, Advanced Care Planning, Managing in the Early Stages, Care Strategies, Driving, End of Life Care and much more

Client Journey can be used for Care Reviews and Accreditation

It's easy to start:

1. Identify the Need and Agree in Client Care Plan

2. Provide MyDC client name, carer name and email address

3. If clients need assistance there are Videos, Tutorials and Demos available. Lots of regular contact and prompts help guide them along the way.
