1. Identify unmet needs
My Dementia Companion CARER tool is suitable for clients or carers with identified changes in cognition that affect memory, attention and other cognitive abilities:

Where cognitive changes are impacting on someone’s activities of daily living, or a carer needs to maintain their health and wellbeing, they may be eligible for support at home

This can include difficulties with activities of daily living or personal care, driving and transport, communication, navigating the home or maintaining the home, carer burnout, financial and legal planning, nutrition and hydration, medication management, mobility and falls, dental or oral care, continence care, social participation and engagement, delirium and changes in behaviours such as refusal, withdrawal, hallucination, sleeping difficulties, repetition, wandering/walking-about, aggression, agitation, hoarding, hiding etc.

The above can be identified through your own professional assessment or My Aged Care assessment team.
