Sample Care/Support Plan text for CARER tool

 

My Dementia Companion CARER tool is suitable for clients or carers with identified changes in cognition that affect memory, attention and other cognitive abilities.

Sample Care Plan text you can tailor as part of the identified unmet needs and agreed support in Care/ Support Plan:

  • My Dementia Companion will enable [client name] to gain practical guidance and support, with a re-ablement approach, to remain safe and independently on [his/her/their] dementia journey at home. [Client name] can access the guidance at their fingertips at any time, including understanding dementia, communication, changed behaviour strategies, carer support, environmental changes, respite care and much more [tailor this to the needs of client].

Goal: Drew’s cognitive skills are declining, and she would like to have access to some simple strategies to cope with changes at home.

Progress Note: Socrates's wife Marge rang distraught as she had been calling around to get information on dementia to know how to care for Socrates after his recent diagnosis.

 

Marge said the process caused her a lot of stress and stated she did not want to talk to anyone on the phone anymore.  Marge reported that she would prefer to be able to access information when she needed it in small pieces as she felt daunted about their future. The pamphlets she currently has and searching online is overwhelming. She wanted a resource where she could access solutions for her husband, keep track of where they were at and know what was ahead of them.  I advised her that My Dementia Companion would be perfect for her and it could be included in Socrates's Package.  I asked if she would like me to send her information about the platform. Marge agreed and would let me know if she needed help or would like to proceed and include the tool in Socrates's Home Care Package. I could update Socrates's Care Plan and Budget to reflect the agreed change once she decides.

Reese Withoutspoon

Transport, Mobility and Driving

 

Goal: Reese doesn’t drive as much as she used to but still wants the option to get to appointments on her own as her husband still works. She would like flexibility to use other forms of transport if she can’t drive.

Agreed Service and Support: Reese would like to access My Dementia Companion CARER Tool through her package to source information, resources, solutions and suggestions to enable her to remain independent from her husband for as long as possible.

Lindsey Hihand

Dementia support

 

Goal: Lindsey has lost decision making capacity and is fully reliant on her husband Snoop to provide all her care and support.  Snoop feels overwhelmed and would like a resource that will help him cope with the changes as they occur.

Agreed Service and Support: By providing Snoop access to the My Dementia Companion CARER Tool, through Lindsey's package, he will be able to source information, resources and care strategies when he needs them (at a time that fits with his caring role) to care and support Lindsey, so Lindsey can live well at home safely and as independently as possible.  

Clint Westwood

Personal Care and Daily Activities

 

Goal: Clint wants to manage his personal activities living with dementia remaining independent for as long as he can.  He wants to retain choice and control and reduce the strain on his family.

Agreed Service and Support:
Clint would like to access My Dementia Companion CARER Tool through his package to source information to understand his dementia, where he is at in his dementia journey and strategies or solutions they can put in place to help at home.  Clint wants the choice to plan for his future and learn to manage upcoming changes himself, while he is able to.
